A manufacturing company in Oldcastle, Tecumseh has been ordered to pay $185,000 after a worker died in an incident in August 2024.

The fatality occurred at Integrity Tool and Mold Inc., a business that produces moulds for the automotive sector. The worker had been servicing a CNC machine, which is a computer-controlled device used to cut and shape materials. While the machine was in manual mode, a program was activated that switched it into automatic operation.

During the repair, the worker entered the machine’s enclosure and suffered fatal injuries when it became active.

An investigation by the Ministry of Labour found multiple safety failures. The machine had not been shut down or properly locked out, the worker lacked adequate lockout training, and no locks were supplied. Inspectors also discovered incorrect labeling on the machine’s lockout points and the absence of a required lockout placard.

After pleading guilty in a Windsor court, the company received the fine along with an additional 25 per cent surcharge, which is directed to a provincial fund supporting victims of crime.
